What is a Geriatric Care Manager?

Our Geriatric Care Managers are licensed social workers who are ready to support and guide your loved one as they require—everything from navigating insurance and making sure bills are paid to making travel arrangements and planning social outings. 

We understand that managing daily life can become a lot, which is why we offer a helping hand with tasks like:

  • Reviewing mail, keeping bills current, and home organization: We can help stay on top of paperwork and keep your loved one’s living space tidy and functional.
  • Emotional support & crisis management: Life has its ups and downs. We’re here to offer a steady presence and help navigate difficult moments.
  • Assistance locating and coordinating with service providers: Whether you need a healthcare provider, a trusted cleaner, a handyman, or even professionals like attorneys or accountants, we can help you find and connect with the right people.
  • Reviewing and maintaining follow-up medical appointments & coordinating with providers: We’ll help keep track of important appointments and communicate with your medical team to ensure continuity of care.
  • Additional communication and oversight for you and your family: We can act as an extra set of eyes and ears, keeping you and your family informed and connected.
  • Navigating insurance and benefits: We can help you understand and manage your Long Term Care Insurance and Commercial Insurance benefits and reimbursements, as well as assist with Veterans Benefits & Reimbursements.
  • Travel arrangements: For peace of mind, we can assist with state-to-state travel by plane or ground.
  • Finding the right living situation: We offer assistance finding an Assisted Living or Skilled Nursing Facility that best suits your needs.
