Care Plan / Fall Prevention
Our process allows us to create a care plan tailored to you or your loved one’s needs. With an open dialogue, we gather as much information as possible that allows us to make a personality and clinical match when selecting an appropriate caregiver. We will discuss a range of care needs, health history, background, and personality questions.
We will work together to make sure the home environment is safe and conducive to your loved one’s recovery. An “ADL” log will be present in the home which includes the care plan, daily check lists, care notes, emergency contacts, and useful tips for many situations and needs.
Every 11 seconds a senior is treated at the emergency room for a fall. Falls are common among seniors, particularly those with cognitive impairment or history of falls in the post-discharge period. Fall related injuries are the leading cause of hospital readmissions.
If you or your loved one is at risk for falling, The Senior Company’s Registered Nursing Staff will make sure that the care plan created includes a history of falls and how our matched caregiver will be directed to provide focused safety at home. Our Registered Nurses will visit your home as well to make any necessary safety recommendations to ensure that the risk of falling is dramatically reduced.
